The Yolo County Probation Department, in collaboration with the County’s partnerships and community-based organizations who support behavioral health improvement, is seeking to provide services to clients on active probation supervision to transition them from recent incarceration, homelessness, and/or experiencing substance use disorder and mental health needs, with a three-pronged approach. The first goal of the Transitions to Care (T2C) program is to improve behavioral health of participants with identified needs through engagement in appropriate services and reduced risk/harm related to ongoing substance use and to address needs of participants experiencing mental health crises. The secondary goal is to increase the self-sufficiency of participants through secured stable housing, assistance with rental navigation, and enrollment in health insurance. The final goal is to reduce the criminal justice system involvement for participants through prevention, diversion, and restorative justice.
The Probation Department applied for and was awarded a $2.0M grant by the Board of State and Community Corrections that will be used to expand its Transitions to Care program in the community. This award allocates 65% of the funds to community-based organizations to identify and fund case management services, substance use disorder counseling and treatment, housing navigation services, and stipends for housing placement. The remainder of the funding is being utilized by Probation to provide community supervision, logistical support, and data collection and reporting for the BSCC.
The Probation Department has contracted with a local treatment provider, Cache Creek Lodge, to help provide behavioral health treatment and long-term stable living. Cache Creek Lodge services include residential treatment, transitional housing, and employment navigation, all aimed at improve long term successful outcomes for those who enroll and successfully complete the program. Additionally, the Probation Department has created a Local Advisory Committee (LAC) to help identify and prioritize how to address clients’ individual needs. The LAC will meet on a regular basis to discuss progress, milestones and potential concerns. Meetings will be posted beforehand to allow the community to attend and participate in the discussion. The meetings will follow Brown Act regulations to provide ample opportunity and transparency to the public.
