Kindergarten Oral Health Assessment (KOHA)

The Kindergarten Oral Health Assessment (KOHA) requirement was passed into law under the Education Code Section 49452.8 in 2005 by Assembly Bill 1433 (AB 1433). California state law requires children to receive a dental assessment by May 31st in either Kindergarten or first grade, whichever is their first year in public school. This helps ensure children receive the oral health care they need to be healthy and ready for school. The KOHA form is distributed when you register your child for school, or you can download the form below.

System for California Oral Health Reporting (SCOHR)

DATA COLLECTION & IMPUTTING KOHA DATA INTO SCOHR

SCOHR is a centralized online reporting system to report and tract the status of student oral health assessment data developed by the San Joaquin County Office of Education (SJCOE). Each school is required to track, tally, and report specific data from the KOHA forms for each child eligible for KOHA. The forms are collected and tracked continuously throughout the school year.

The data is reported into SCOHR at the end of the school year or by July 1st at the latest.

Users can input, upload, manage and export KOHA forms and data suing the SCOHR system at school, district, county, and state levels.

Data Input Forms

In SCOHR, you can input your data by signing into SCOHR and choosing from two options:

1. Enter aggregate data for one school at a time using the Data Input Form. You can improve your data collection when entering data into SCOHR this way by using the Excel spreadsheet on the right. The video at the bottom of this page explains how to use the Excel spreadsheet for Data Input Form entry.

2. Upload aggregate data for multiple schools at a time using the bulk upload function If the Excel spreadsheet above is not used, an alternate data entry aide is the PDF fillable form to the right, where aggregate totals can be entered for the school. It color codes data line items, and maps them on the second page to where they can be found on the actual KOHA forms.
