We are pleased to share with you the 2026-2028 Community Health Improvement Plan (CHIP). The CHIP outlines Priorities, Goals, Strategies, and Activities to address significant health needs. The CHIP Executive Summary is available here.

 CHIP development occurred in two phases between 2023-2026. For those interested in the history of CHP development, the Phase I CHIP document that includes development of the CHIP Priorities and Goals can be found here. The 2024 CHIP annual report provides an update on progress between Phase I and Phase 2 can be found here.

The CHIP is built upon the significant health needs found in the 2023-2028 Community Health Assessment (CHA).
