YOLO COUNTY RESIDENTS TO HELP WRITE COMMUNITY HEALTH IMPROVEMENT PLAN

Eight Yolo County residents have joined a new Community Advisory Program (CAP) to help write 2023-2026 Yolo County Community Health Improvement Plan (CHIP)

Community Advisors come from the communities of Davis, Esparto, Guinda, West Sacramento, Woodland, and Zamora.

PART 1. PREPARE: Two CAP learning sessions

September 2022 to October 2022

One hour sessions for Community Advisors to learn about the Public Health Department and CHIP

PART 2. DEVELOP: Community Advisors join Workgroup to write CHIP

October 2022 – May 2023

Community Advisors join representatives from government, healthcare, and community-based organizations to develop the CHIP by:

Identifying priority locations and communities
Selecting county's top health needs to work on
Building goals for improving top health needs
Developing strategies to achieve goals

PART 3. ACT: Put CHIP strategies into action and track progress

June 2023 – June 2026

Participate in four action meetings each year and directly help get projects going (if applicable)

FAQs

Who is eligible to participate in the Community Advisory Program (CAP)?

Participants must live in Yolo County and be at least 14 years old. They cannot also represent an organization that will be participating in the CHIP Workgroup.
How will Community Advisors be paid?

CAP participants will be vendors and not Yolo County employees. Advisors must complete a vendor agreement to receive the stipend. The stipend rate is $25 per hour for participating in meetings and for up to 2 hours per month of research or review time. The stipend will be given after final CHIP development (May 2023).
What is the difference between the CAP and the CHIP Workgroup?

The CAP is only for Community Advisors (residents). It includes two learning sessions before CHIP development for residents with deep community knowledge and passion for helping the community who may not traditional public health experience. The CHIP Workgroup includes people from county government, health care agencies, and non-profit community groups. Community Advisors will participate in the CHIP Workgroup as paid representatives of the community.
Do I have to go through the CAP to participate in the CHIP Workgroup?

Joining the CAP is not required for community members to participate in the CHIP Workgroup. However, it is a paid opportunity for residents to build relationships with each other and learn more about the CHIP.
What are you looking for in a Community Advisor?

People of all ages and experiences with deep knowledge of their neighborhood and city/town. Educational or professional background in public health is NOT needed.
How will Community Advisors be selected?

CAP is designed for up to ten Community Advisors, ideally two advisors from each urban areas (Woodland, Davis, West Sacramento, and Winters) and two advisors from rural/Unincorporated areas. If more than ten people complete interest forms, Advisors will be selected based on ability to attend all meetings and representation of populations experiencing health disparities.
When and where are the CAP meetings?

Meeting location and time have not been determined (may be virtual or in person). Advisors will be asked for their preferences before start of the CAP.
When and where are the CHIP Workgroup meetings?

Meeting location and time have not been determined. Some will be in-person and some will be virtual. Meetings occur once a month and for two hours.
Do I need a computer and internet?

Yes
Will transportation costs be covered?

No
Will you provide childcare?

No, though children are welcome to attend with their caregivers.
Is this my only chance to get involved with the CHIP?

No! A draft CHIP will be presented at a townhall for community members to review before it is finalized.
What if I don't speak English fluently?

Interpretation services can be provided to anyone requesting them.
What if I miss a meeting?

We understand if you need to miss a meeting. Missing two or more will require a conversation about whether you are able to commit participation at this time.
